PLAYER INFORMATION, MEDICAL TREATMENT AUTHORIZATION, LIABILITY WAIVER/RELEASE AND CONSENT FORM (2024-25 REGISTRATION YEAR)

The parent/guardian (in the case of a minor player) or adult player is to complete this form as part of the Legends Rec League player registration process. Form #R002 must be completed on an annual basis.

This form is to be retained by the Legends Recreational Soccer League for at least five (5) years or until the player’s 18th birthday, whichever occurs last.
